Appointment Scheduling

When scheduling your appointment, please provide your insurance plan and ID numbers and obtain your referral if needed.

Pre-Registration

New patients may access registration forms on our site and complete them prior to arriving for your appointment. Established patients are asked to access the Patient Portal and update any medications, medical history, etc. prior to your arrival.

Check-In

Provide the patient services representative with your pre-registration forms along with your referral if we have not received it prior to today. Pay your co-pay at check-in also. Medical history, etc. will be obtained for new patients prior to seeing the physician, so please bring your detailed list of medications with dosages/frequencies.

Laboratory Services

Our practice has an in-house dermatopathology laboratory at our Grant Road location; Dr. Scott Bangert is Board Certified in this specialty area. The on-site lab allows us to oftentimes expedite the results and process some specialized tests more conveniently, if needed. If during your visit a diagnostic test requires lab processing, you will see a separate charge(s). Occasionally we are required to send specimens to an outside lab and it is accompanied with your insurance. In this situation you will receive a separate bill or explanation of benefits.

Insurance

If your insurance should change, you need to notify us of such changes; otherwise, you may be responsible for your charges. If we are not contracted with your secondary insurance or if Medicare does not forward it to your secondary insurance, you will be responsible for submitting such claims.

No-Show/Cancellation Policy

Upon scheduling an appointment, we have set aside time for you. In return, if you cannot make you appointment we require appropriate notice. Therefore, effective January 1, 2019, “no show” appointments and cancellations with less than 24 hours notice (or not cancelled by noon on Friday for Monday) will be charged a fee of $40.

We encourage you to provide us with your primary care physician information so that we may forward your visit notes or other pertinent information about your care as needed.
